What is a Lingual Holding Arch and its clinical purpose?

A fixed lingual arch on the lower arch used to maintain space and control eruption of posterior teeth

A Lingual Holding Arch is a fixed space-maintaining appliance placed on the lingual (tongue) side of the lower arch. It attaches to the mandibular first permanent molars with bands and connects them with a wire that runs along the lingual surfaces. Its clinical purpose is to keep the molars from drifting mesially so there is enough space for the eruption of posterior teeth (premolars and canines) and to preserve overall arch length during mixed dentition. It is not used to widen the arch or to correct tongue thrust, and it is distinct from palatal appliances that sit on the palate.

A removable palatal plate to widen the arch

A device on the palate to hold molars in place

A device to correct tongue thrusting
